Is it worth replacing a missing back tooth?

Yes. Missing back teeth cause a chain reaction: adjacent teeth shift, opposing teeth over-erupt, bone deteriorates, and chewing efficiency drops significantly. What starts as one missing molar can lead to multiple dental problems. Replacing it early after extraction is simpler and cheaper than fixing the consequences later.

Can I just leave the gap if nobody can see it?

Having priced up the same implants on the South Coast I found Dental by Design a lot cheaper . I had my whole top jaw teeth replaced by Dr. Mechoui having been worried by the process I found him and the surgery excellent at making me feel relaxed and informed at all stages.
