Referral Program Terms and Conditions

1. Introduction and Acceptance

These Terms and Conditions ("Terms") govern your participation in the Dental by Design Patient Referral Program ("Program"). By participating in the Program, you acknowledge that you have read, understood, and agree to be bound by these Terms.

The Program is operated by Dental by Design ("we," "us," or "our"), located at our practice premises. These Terms constitute a binding agreement between you and Dental by Design.

2. Eligibility Requirements

2.1 Referring Patient Eligibility

• • You must be an existing patient of Dental by Design who has received treatment within the past 18 months
• • You must be 21 years of age or older
• • You must have a clear account with no outstanding balances
• • You must be in good standing with our practice
• • You must not be an employee, contractor, or immediate family member of an employee of Dental by Design

2.2 Referred Patient Eligibility

• • Must be a new patient who has never been treated at Dental by Design
• • Must be 21 years of age or older
• • Must be seeking dental implant or Invisalign treatment
• • Must book their initial consultation within 90 days of referral

3. Reward Structure and Redemption

3.1 Reward Details

Rewards are earned when your referred patient starts their treatment. Vouchers are issued only after treatment has commenced.

• • Reward vouchers based on number of referrals: £95 (1st), £175 (2nd), £395 (3rd+). Full arch referrals receive £395 immediately. No limit
• • Vouchers can be used for any dental treatment or service at our practice
• • Rewards are credited within 48 hours of the referred patient starting their treatment

3.2 Redemption Process

• • Rewards must be redeemed within 6 months of issuance
• • Appointments must be scheduled through our standard booking process
• • Rewards cannot be combined with other offers or promotions
• • Missed appointments without 24-hour notice will forfeit the reward

4. Program Limitations and Restrictions

• • Rewards have no cash value and cannot be transferred, sold, or exchanged
• • Self-referrals are not eligible for rewards
• • Referrals must be made through official program channels
• • Retroactive referrals (patients who have already booked their appointment) are not eligible
• • Rewards cannot be applied to past or existing treatment plans
• • Program participation may be revoked for violation of these Terms

5. Privacy and Data Handling

5.1 Information Collection

We collect only the minimum necessary information to administer the Program, including:

• • Your name and contact information
• • Referral tracking information
• • Reward redemption history

5.2 Data Protection

• • All personal information is protected under GDPR and applicable UK privacy laws
• • Data is encrypted and stored securely
• • We do not sell or share your information with third parties for marketing
• • You may request access to or deletion of your program data at any time

5.3 Communications

By participating in the Program, you consent to receive program-related communications. You may opt out of marketing communications at any time while remaining in the Program.

6. Program Modifications and Termination

6.1 Modifications

• • We reserve the right to modify these Terms at any time without prior notice
• • Changes will be posted on our website and communicated via email where reasonably practicable
• • Continued participation constitutes acceptance of modified Terms
• • Accrued rewards remain valid under original terms unless otherwise stated

6.2 Termination

• • We may terminate or suspend the Program at any time with 60 days notice
• • Individual participation may be terminated for violation of Terms
• • Upon termination, unredeemed rewards must be used within 90 days

7. Dispute Resolution

7.1 Dispute Process

• • All disputes must first be addressed through our patient relations team
• • Written notice of disputes must be provided within 30 days of the issue
• • We will respond to disputes within 10 business days

7.2 Arbitration

Any disputes not resolved through our internal process shall be resolved through binding arbitration in accordance with UK arbitration rules. The arbitration shall take place in London, United Kingdom.

8. Legal Compliance

8.1 Regulatory Compliance

• • This Program complies with all applicable UK healthcare regulations
• • Rewards are based on general patient loyalty, not tied to specific treatment volume or value
• • The Program does not constitute payment for referrals under anti-kickback statutes
• • All aspects comply with UK Bribery Act and General Dental Council guidelines

8.2 Tax Implications

Participants are responsible for any tax obligations arising from Program rewards under UK tax law. Consult your tax advisor for specific guidance.

9. General Provisions

9.1 Entire Agreement

These Terms constitute the entire agreement between you and Dental by Design regarding the Program and supersede all prior agreements and understandings.

9.2 Severability

If any provision of these Terms is found to be unenforceable, the remaining provisions shall continue in full force and effect.

9.3 Governing Law

These Terms shall be governed by the laws of England and Wales, without regard to conflict of law principles.

9.4 Waiver

Our failure to enforce any provision of these Terms shall not constitute a waiver of that provision or any other provision.
